Patient Forms
ALL patients (both brand new and previous Moore Pediatrics patients) must complete patient registration forms prior their first appointment with Sunrise Pediatrics.
We kindly ask that you complete all fields in full.
Please complete this form if you are needing to request records from a previous primary care clinic, specialist, urgent care, emergency department OR if you are needing Sunrise Pediatrics to disclose your child's medical records to a new office for transfer of care.
*If you are a former patient of Moore Pediatrics and need to request your past medical records, please click here to be redirected to Norman Regional's website where you can submit your request.
Please bring this form to your scheduled Sports Physical appointment. All parent/patient related questions (page 2) must be completed upon arrival.

*Sports Physicals are a flat fee of $25.00, due at the time of service. If your child is due for his/her wellness exam and also needs a sports physical completed, you may have this form filled out during the WCC appointment free of charge.
ALL patient information & parent questions on any form MUST be completed by the parent prior to the child's appointment.

This includes: patient name and DOB, medication information, patient history questions, etc.

Our Policy on Vaccinations

We follow the immunization schedule recommended by the Centers for Disease Control and Prevention (CDC) and the American Academy of Pediatrics (AAP).

At the same time, we respect a parent’s right to make medical decisions for their child. We welcome open conversations about vaccines and are happy to discuss any questions or concerns so we can work together on a plan that feels safe and informed.

Because emotional wellness matters too, we routinely screen for depression in patients ages 11+ during yearly well visits to help support healthy minds and healthy lives.
